Responsibilities As a Social Services Designee you will assist in preparing our residents for their future. You will ensure that residents receive a high standard of care by meeting their social and emotional needs during their stay, coordinating resident discharges, and participating in resident care conferences.
Qualifications As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role, preferably in a healthcare setting. Prior Social Services Designee experience in a Skilled Nursing Facility, a bachelor’s degree in Social Work or a similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate.
