What You’ll Do:
Conduct initial assessments to understand clients’ needs, strengths, and challenges.
Develop and implement individualized service plans to help clients achieve their goals.
Provide ongoing support, advocacy, and guidance to clients throughout their journey.
Connect clients with appropriate community resources, programs, and services.
Maintain accurate and confidential case records, ensuring compliance with organizational policies.
Collaborate with colleagues and external partners to deliver comprehensive support.
Monitor client progress and adjust plans as needed to ensure positive outcomes.
What We’re Looking For:
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience).
Strong interpersonal and communication skills, with a genuine desire to help others.
Ability to manage multiple cases and prioritize tasks effectively.
Proficiency in basic computer applications (e.g., Microsoft Office, case management software).
Familiarity with community resources and social service programs is a plus.
A valid driver’s license and reliable transportation.
Eagerness to learn and grow in a fast‑paced environment.
