Job Summary:
The Providence Center Residential Case Manager Women's Road to Recovery provides clinical case management services to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address mental health symptoms and co-occurring disorders. Work with consumers to achieve the highest level of independence possible.
Duties and Responsibilities:
Initiate treatment and advocate for the consumer within the network of care. Assist in the development and implementation of socialization and recreational activities within the group home and community, including attendance at Day Program groups. Complete recordkeeping in accordance with Health Information Services (HIS) and Center requirements, maintaining confidentiality. Provide case management and support, make assessments and deliver appropriate crisis interventions and risk management to maintain a calm and safe environment. Teach consumers adult daily living skills and assist them in all aspects of their life in the group home and supervised apartments. Transport consumers in their own vehicle or Center van to planned activities and appointments. Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
Requirements:
High School Diploma/GED required. Experience in residential facilities preferred. Must have a valid driver’s license, registration, current inspection, and an insured automobile.
