Scenic Wellness Nursing & Rehabilitation is seeking a compassionate and dedicated Social Services Director to lead our Social Services department. In this role, you will help create a supportive, person-centered environment that promotes dignity, independence, and quality of life for our residents.

What You’ll Do
Oversee and manage all Social Services operations in accordance with NASW Code of Ethics and all federal, state, and local regulations.
Foster a positive, resident‑centered climate that supports individuality and psychosocial well‑being.
Complete psychosocial histories, assessments, and relevant MDS sections; use MDS data to support care planning.
Participate in care planning and collaborate with the interdisciplinary team to address psychosocial needs.
Monitor and support resident rights, mental health, and adjustment to the facility.
Maintain accurate, compliant documentation (care plans, progress notes, behavior tracking, PASARR, discharge planning, etc.).
Educate and support residents and families through placement, adjustment, discharge planning, and access to community resources.
Coordinate the discharge planning process, referrals, and post‑care arrangements.
Assist in policy development and annual survey readiness.
Perform additional duties as assigned.
What We’re Looking For
Certified, licensed, or registered Social Worker in the state of Missouri (required).
Bachelor’s degree in Social Work or Human Services (e.g., Sociology, Psychology, Special Education, Rehabilitation Counseling) required.
At least 1 year of supervisory social work experience in a healthcare setting working with geriatric populations.
2+ years of skilled nursing facility experience preferred.
Strong leadership, communication, and assessment skills.
