What You’ll Do
Develop, plan, implement, and evaluate social service programs in accordance with state and federal regulations
Identify and address residents’ social, emotional, and psychological needs
Provide service coordination for residents and families
Support resident rights, dignity, and overall well‑being
Collaborate with interdisciplinary team members to ensure comprehensive care
What You Bring
Licensed Social Worker preferred (not required)
Minimum of one year of supervised social work experience in a healthcare setting (experience with elderly populations preferred)
Working knowledge of nursing home regulations, including MDS procedures and Medicare/Medicaid eligibility and reimbursement guidelines
Excellent written and verbal communication skills
Positive, dependable, and team‑focused attitude
Strong attendance and professionalism
