The

Community Liaison

is a vital connector between our healthcare services and the community. This role drives

business development, outreach, and relationship-building

with referral partners, patients, caregivers, and the broader community. What You’ll Do

Build and maintain strong relationships with referral sources (hospitals, physician offices, senior living communities, healthcare providers). Identify new business opportunities to grow patient census across service lines. Represent the Hospice at community events, health fairs, and networking functions. Patient & Family Engagement

Educate patients, families, and caregivers on hospice services and eligibility. Serve as a compassionate resource to guide families through the enrollment process. Communication & Coordination

Facilitate smooth communication between referral sources and clinical teams. Ensure timely follow-up on referrals and accurate documentation of outreach. Market Leadership

Act as the face of the Hospice in your community, building trust and awareness. Monitor market trends and competitor activity to inform strategy. What We’re Looking For

Proven experience in healthcare marketing, sales, or community outreach (hospice, home health, or related field preferred). Strong track record of meeting/exceeding referral and census growth goals. Excellent interpersonal and communication skills, able to connect with diverse audiences. Knowledge of Medicare/Medicaid, hospice eligibility, and CRM systems a plus. Ability to work independently and collaboratively in a fast-paced environment. Valid driver’s license and reliable transportation required. Why Join Us?
