DAY HABILITATION/COMMUNITY HABILITATION

Job Summary The Day Habilitation/Community Habilitation R Provider serves as a role model and advocate, ensuring individuals' safety by providing consistent supervision and observation, and promoting a nurturing learning environment in the Day Habilitation Without Walls, the Community, and on-site in Dominical Hall. The Provider implements individuals' Comm Hab R Plans and provides training of adult living skills in a dignified and respectful manner. Duties & Responsibilities Ensure individuals' safety by providing consistent supervision and observation. Assist in the development, implementation and documentation of Individual Habilitation Plans, Life Plans and Day/Community Habilitation Plans. Execute tasks on‑site and in the community, training individuals with developmental disabilities in various life skill areas to promote fuller personal development, enjoyment, and expanded independence, community integration, productivity, and individualization. Successfully complete AMAP Certification within 90 days of hire. Administer medications as prescribed within 90 days of hire. Complete training: tube feeding, diabetic, colostomy, and any specialized certification required, within 90 days of hire. Education & Experience High School Diploma or GED. Valid (non‑restricted) Driver's License. Physical Requirements One year of experience with developmental disabilities. Must stay awake during the entire shift; sleeping on the job is prohibited. Must communicate clearly and effectively with staff and individuals. Perform SCIP‑R physical interventions (e.g., one‑person escort, standing wrap, seated wrap, one‑person take‑down onto side) as necessary. Must be able to travel stairs. Must be able to perform one‑person transfers. Must be able to lift a minimum of 25 pounds using lifts where appropriate and determine when assistance is needed. Seniority level
