Community Care Social Worker/Case Manager (Full Time)

Availability & Work Arrangement
Monday through Friday – remote option (must live in Yellowstone or surrounding counties)
No call or weekend shifts

Works in a multi‑disciplinary approach to plan, coordinate, monitor, and supervise the provision of services to consumers enrolled in the Home and Community Based Services Program.
Key Responsibilities
Plan, coordinate, and monitor services for Home and Community Based Services Program participants.
Handle pressure, meet deadlines, maintain accuracy, and adapt to changing situations.
Interact with a variety of people, manage stressful situations, and resolve conflicts.
Perform assigned job duties, adhere to licensing board regulations, and follow Benefis policies and procedures.
Education / License / Experience Requirements
Bachelor’s degree in social work, psychology, sociology, or a related field.
Three (3) years of social work experience in a health care setting (preferred).
Knowledge of case management methods, diagnostic and crisis intervention skills.
Knowledge of issues and needs of long‑term care consumers.
Experience managing a budget.
Understanding of human behavior, disabilities, and the aging process.
