Major Responsibilities
Assists Psychiatry in refilling medication for CHL clients by answering the Medication Refill Line and communicating and educating clients about medications.
Provides prior approvals for medication prescriptions as needed by insurance companies.
Completes nursing assessments of clients screening for, but not limited to, such diseases as diabetes, hypertension, and tuberculosis.
Identifies medical, emotional, and social problems requiring further assessment and treatment.
Provides appropriate health care and coordinates referrals as needed.
Facilitates referrals to primary care provider or establishes contact with existing primary care provider.
Contributes to the development of treatment plan, particularly as related to medical concerns. Establishes realistic goals, documents clear and concise plans of care, regularly updates plans of care based on change in health status. Plan of care is made with client, family, and team members.
Works with individual clients on wellness initiatives such as smoking cessation, improved nutrition, increased physical activity, and stress management, and follows through with all medical care recommendations.
Facilitates educational groups regarding nutrition, physical activity, smoking cessation, stress management, and medical needs for clients. Provides educational support to increase knowledge of clients and mental health providers regarding medical conditions. Plans and facilitates wellness activities.
Maintains regular contact among consumer, primary care, and primary and psychiatric providers, using existing family and social supports as well.
Records all assessments, treatment plans, and follow up data in the electronic medical record system daily.
Specialty Responsibilities – CBHC Community RN
Nursing assessments may be completed in the community, including in schools and homes.
Completes nursing assessment updates.
Completes assessments and updates consistent with required timeframes.
Standard Staffing Level Responsibilities
Complies with established departmental policies, procedures, and objectives.
Attends variety of meetings, conferences, seminars as required or directed.
Demonstrates use of Quality Improvement in daily operations.
Complies with all health and safety regulations and requirements.
Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
Maintains, regular, reliable, and predictable attendance.
Performs other similar and related duties as required or directed.

Position Qualifications License/Certification/Education
Required: Valid and current license as a Registered Nurse in the Commonwealth of Massachusetts.
Required: Current valid US‑issued driver's license.
Preferred: Bachelor’s degree in nursing.
Experience/Skills
Required: Experience in the area of practice is necessary.
Required: Demonstrates person‑centered strength‑based communications and relationship with consumers using recovery‑oriented principles.
Required: Demonstrates proficiency in standard nursing practice including assessment, intervention, and evaluation.
Required: Demonstrates ability to communicate effectively and professionally with consumers, staff and providers.
Required: Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
Required: Strengths/Experience in Wellness oriented care plans including nutrition, physical activity, tobacco cessation, and stress management.
Preferred: N/A.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Physical Demands and Environmental Conditions Work is considered medium. May need to lift up to 25 pounds.
Work is in an indoor patient‑focused environment.
Reliable vehicle, as this position requires transportation of clients for medical appointments.
