Job Summary Position delivers housing-focused case management and supportive services to eligible individuals and families living with HIV/AIDS, advancing housing stability, access to care and self-sufficiency in alignment with HOPWA grant requirements.

Job Purpose Position helps people living with HIV/AIDS obtain and maintain safe, stable housing so that they can better engage in medical care and work toward self-sufficiency. The role exists to translate HOPWA funding and guidelines into direct, practical support for clients through assessment, housing assistance, and coordinated services. It also ensures the organization meets its contractual and compliance obligations under the HOPWA grant by providing timely, well-documented case management and housing interventions.

Essential Functions Key accountabilities include client assessment and planning, housing services and service coordination, caseload management and client engagement, and compliance and reporting.

Job Factors Minimum Education Level Required:

Bachelor’s degree in social work, psychology, human services, public health or a closely related discipline.

Minimum Experience Level Required:

Minimum two years of HIV/HOPWA-specific case management experience or three years of progressively responsible experience in housing services, case management or grant-funded program administration.

Knowledge, Skills and Abilities

Deep knowledge of HIV/AIDS, related stigma and psychosocial issues impacting people living with HIV/AIDS including how housing affects health outcomes.

Demonstrated knowledge of HOPWA and HUD basics, including STRMU, TBRA, PHP, fair housing, tenant/landlord rights, confidentiality and HIPAA requirements.

Deep connection to local housing resources, benefits systems, and community-based medical, behavioral health and social service providers.

Strong assessment, care planning and service coordination skills.

Excellent time management, prioritization and problem-solving skills to manage a caseload, meet deadlines and respond to housing crises.

Strong written and verbal communication skills with the ability to communicate effectively with all levels of the organization and adapt messaging for the appropriate audience. Bilingual preferred.

Ability to build supportive, professional relationships while maintaining boundaries, cultural humility, and nonjudgmental, trauma-informed practice.

Ability to function in a complex team environment, collaborating effectively with multidisciplinary teams to meet needs.

Self-directed and self-motivated, demonstrated ability to meet deadlines and manage competing demands efficiently and simultaneously.

Strong documentation and data entry skills; able to navigate Microsoft Office products and EHR/HMIS or similar databases.

Strong critical thinking and adaptability in a fast-changing, highly regulated environment.

Ability to work independently with minimal supervision, meeting hard deadlines.

Physical Demands

High mobility, prolonged standing and walking, sometimes for extended periods without breaks.

High manual dexterity and repetitive movements using hands and fingers.

Motor vehicle and other transportation operations between campus locations, client home/community visits and to other off-campus locations and events (Travel 25%).

Lifting items up to 30 pounds and physical exertion.

Frequent bending, stretching, twisting and reaching.

Access to information using a computer.

Environmental Conditions Indoors in an environmentally controlled space with exposure to environmental hazards (contaminants, hazardous equipment, and infectious disease). Occasional work outdoors exposed to elements. Frequent performance of position duties while standing and walking and otherwise in motion. Frequent computer use at a workstation up to two hours or more at a time. Frequently fast-paced work with unscheduled interruptions. Work may require an adjusted work schedule, overtime, and evening/weekend hours in order to meet patient care demands. Some public contact requiring business-appropriate apparel and/or occasional use of personal protective equipment.
