POSITION SUMMARY: The Community Relations Liaison, in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of the organization to current and potential referral sources, patients and their family members, and the community at large. The Community Liaison will provide all necessary education to the patient, their family members or caregivers, and facility staff regarding the patient’s progress toward the goals outlined in their plan of care and other areas deemed necessary to provide the highest quality care to the patient.

REPORTS TO: Director of Operations or Executive Director

SUPERVISES: None

QUALIFICATIONS:

Education: Bachelor’s degree in a healthcare-related field is preferred.

Credentials: None

Experience: 2+ years of hospice, palliative, home health, or similar experience is preferred.

Core Competencies: Working knowledge of applicable Medicare, Medicaid, and Private Insurance regulations and guidelines. Excellent verbal and written communication skills; must be able to respond to the needs of providers, discharge planners, facility staff, patients, and family members. Proficient in observing and reporting changes in patient status, and providing accurate, comprehensive, and timely reports to the care team. Experience in developing effective marketing strategies is strongly preferred.

Other: Valid driver’s license and current automobile insurance.

FUNCTIONS & RESPONSIBILITIES:

Work with the admission team. Meet with facility staff and community members to establish presence within the territory. Assist with obtaining admission documents and patient financial information. Establish presence in the field to promote organization services. Respond to referrals for evaluation and/or admission. Verify and complete all admission and financial documentation. Explain the program to patients, families, and referral sources. Coordinate referrals, admissions, and care needs with the admission team. Identify key referral sources and assist in developing marketing plans. Participate in in-services, staff meetings, and case conferences. Complete documentation accurately and legibly. Adhere to policies, laws, regulations, and standards. Perform other duties as necessary. Seniority level
