We are looking for qualified individuals to join our Guided Personal Support Program as a Community Support Coach in West Newbury, MA and the surrounding area. Schedule:

Per diem, 1.5 to 10 hours a week, based on client needs and availability. Pay Rate:

$20-$25 hourly based on education and experience. The Guided Personal Support (GPS) Program is made up of a combination of SSQUAL contracts (Enhanced Family Support for Adults) from the Massachusetts Department of Developmental Services (DDS) that focus on preparing individuals on acquiring skills in different aspects of their lives to be more independent and live their best life. All the referrals in this program come from DDS. Community Support Coaches are intended to promote additional skill building around community integration, life skills, safety skills. and social skills based on individual goals. The Community Support Coach role is a flexible and creative position. Job Summary:

The Community Support Coach provides in-home and community support for adults with developmental disabilities. Major Responsibilities: Provides support for individuals in their home and/or within the community. Works on goals and skills acquisition with the individual in areas such as communication, social and recreation, ADL, safety skills while promoting independence, self-determination, and self-advocacy Maintains regular communication with families, Program Director and other professionals involved with the family, when appropriate. Works with families regarding intervention techniques if applicable. Assists individuals and families access resources that might be beneficial. Respects human and legal rights of individuals served. Completes all necessary paperwork such as progress notes, incident reports, time sheets and required DDS progress reports throughout the year. Attends all annual mandatory trainings required by the program as well as Northeast Arc. Qualifications: High school diploma or equivalent. Bachelor's degree in human services or another related field preferred. Minimum 1 year working with an individual with disabilities Strong interpersonal skills and community skills Proficient technology skills for documentation, communicating, and clocking in and out while working Ability to safely access the community Valid driver's license and reliable transportation required.
