LaSante Health Center is seeking a dedicated
HIV Community Liaison
to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV-related issues, and a passion for advocacy.
Responsibilities
Support individuals living with HIV and connect them to care.
Conduct outreach and educational initiatives.
Build relationships with community organizations.
Advocate for patients and address barriers to care.
Provide HIV-related education and resources.
Collaborate with healthcare teams to support patients.
Maintain records and complete assigned tasks.
Qualifications
Experience in community outreach or healthcare.
Knowledge of HIV prevention and support services.
Strong interpersonal and organizational skills.
Bilingual in Haitian Creole (preferred).
