Base Pay Range:

$17.18/hr – $19.21/hr

+$3/hour Differential

Location:

Priest Drive Recovery Health Center, Tempe, AZ (approximately 80% field, 20% office)

Employment Type:

Full‑time, 40 hours per week

Qualifications

GED/High School Diploma with a minimum of 3 years of behavioral health experience, or

An associate’s degree in any field with a minimum of 2 years of behavioral health experience

SMI experience preferred

Case management experience is a plus

Housing experience is a plus

Training provided; additional language differential pay available for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL

Job Responsibilities

Provide direct, face‑to‑face community‑based services to assist clients with obtaining and maintaining residential housing.

Screen, intake, and assess clients to evaluate presenting problems and develop treatment plan goals.

Conduct outreach to engage clients into PSH (Permanent Supportive Housing) services and orient potential clients to the H20 program.

Provide transportation to clients as medically necessary, using a personal vehicle or company vehicle when available.

Coordinate care and maintain ongoing collaboration with clients’ families, behavioral and general medical providers, community resources, and other supports.

Assist clients with connections to primary health services and medical appointment follow‑up.

Provide excellent customer service to clients, internal and external agency stakeholders, and state entities (e.g., probation officers, advocates, residential staff).

Maintain contact with each client at least four times a week (including phone calls).

Support clients during pre‑tenancy (3‑6 months) and tenancy (3‑18 months) phases—help with housing vouchers, lease signing, independent‑living skill development, substance‑use services, and case management.

Manage a caseload of up to 15 clients.

Work Monday through Friday; shifts are 7:30 AM – 4:30 PM or 8:30 AM – 5:30 PM; no on‑call or weekend work.
