Base pay range:

$22.00/hr - $26.00/hr (based on experience)

Position Summary Pyramid Healthcare of Charlotte is seeking a

Housing Specialist (Qualified Professional)

to join our Assertive Community Treatment Team (ACTT). Our ACTT program provides a “hospital without walls” approach, delivering high-intensity behavioral health services directly in the community to individuals living with serious mental illness, substance use challenges, and other social determinants of health. Each ACTT is a multi-disciplinary team offering comprehensive, person-centered care tailored to meet clients’ mental health, substance use, vocational, and housing needs.

Position Overview The Housing Specialist plays a critical role in supporting clients’ recovery by addressing housing stability and related social needs. This position combines case management, treatment planning, and interdisciplinary teamwork to ensure individuals receive the resources and support necessary for long-term success.

Key Responsibilities

Housing & Case Management

Assist clients in securing and maintaining safe, stable housing.

Build and maintain partnerships with local housing agencies, landlords, and community organizations.

Provide transportation for clients to community resources as needed.

Treatment Planning & Support

Develop and implement person-centered treatment plans that address housing, vocational, and recovery goals.

Support clients through discharge planning, continuity of care, and connection to community resources.

Participate in emergency services rotation, including crisis planning and hospital diversion.

Collaboration & Documentation

Engage actively in interdisciplinary team meetings.

Maintain accurate and timely clinical documentation per CARF, DHHS, and LME standards.

Communicate with referral sources, families, and community partners regarding client progress.

Continuous Quality Improvement

Participate in CQI teams, peer record reviews, and data entry (NCTOPPS).

Contribute to program compliance and outcome improvement initiatives.

Qualifications

Education & Licensure

Bachelor’s degree in Social Work, Counseling, Psychology, or a related field required.

Master’s degree in Human Services preferred.

Must meet Qualified Professional (QP) status per 10A NCAC 27G.0104.

Experience

1–3 years of experience working with individuals with severe and persistent mental illness preferred.

Background in housing/vocational rehabilitation and community-based mental health strongly preferred.

Other Requirements

Valid driver’s license and reliable transportation.

Strong interpersonal skills and ability to work effectively within a multidisciplinary team.

Commitment to professional growth, ethical practice, and maintaining appropriate boundaries.
