AHRC Nassau . Various part-time daytime, evening, and weekend shifts are available throughout Nassau County. Community Habilitation is a service provided to individuals with developmental disabilities in their own homes or community settings. The person, working together with their family and Care Manager, will create a support plan that best meets the needs of the individual and their family. Primary duties and responsibilities include, but are not limited to: Supporting and assisting with activities of daily living, including personal hygiene tasks such as bathing/showering, tooth brushing, bathroom assistance, and menses care (if applicable). Also includes dressing, feeding, shopping, housekeeping, and laundry skills. Providing companionship, supervision, role modeling, and instruction while ensuring safety in both home and community environments. Documenting service implementation and individual progress in the Medisked system within 24 hours of service delivery. Supporting individuals to become as independent as possible at home and in their community. Qualifications: High school diploma or equivalent preferred. Prior experience supporting individuals with intellectual and developmental disabilities preferred. Valid and qualified NYS driver’s license, compliant with agency’s insurance standards. Comfortable transporting individuals in your personal vehicle. Excellent communication and organizational skills, with the ability to understand written, oral, and email directives. Access to a web browser to complete required documentation.
