Bilingual Spanish-Speaking Intake Specialist
to join our Intake Department. We are a home care agency providing PCA and
CDPAP services
to the community. The ideal candidate will play a key role in managing referrals and ensuring a smooth transition from intake to patient activation. Responsibilities
Receive and review patient referrals Follow up with patients and MLTCs Coordinate and ensure smooth transition from referral to patient activation Maintain accurate and timely communication via email and phone Qualifications
Bilingual in
Spanish and English
(Required) Proficient in email communication and Microsoft Office applications Strong verbal and written communication skills Detail-oriented with excellent time management and organizational skills Strong interpersonal and relationship-building abilities Education & Experience
High school diploma or equivalent (Preferred) Customer service experience: 1 year (Preferred) Computer skills: 1 year (Preferred) Health insurance Paid time off Additional Requirements
Ability to relocate to

before starting work (Required) Pay: $22.00 - $25.00 per hour
