Care Management Assistant
Join AdventHealth as a Care Management Assistant and help provide excellent patient care, coordinating with physicians, care managers, and other members of the healthcare team.
Schedule Part‑time – 24 hours per week. Choose between three‑shift blocks of Friday, Saturday & Sunday or Saturday, Sunday & Monday.

Job Description
Promote excellent customer service through interactions with patients, physicians, care managers, and the healthcare team.
Collaborate with Care Managers for patient care coordination needs.
Coordinate patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.
Provide and document the delivery of regulatory letters to patients or their representatives in accordance with federal and state regulations.
Arrange patient transportation and follow‑up appointments as requested.
Qualifications
High School Graduate or Equivalent (required).
Strong communication skills and attention to detail.
Ability to work independently and as part of a team.
Pay Range $17.11 – $27.38 per hour.
