CFSP Care Extender (Peer Support Specialist) – Harnett or Robeson County

We are partnering with North Carolina DHHS to operate a statewide Medicaid Plan that supports Medicaid‑enrolled infants, children, youth, young adults, and families served by the child welfare system with seamless, integrated, and coordinated health care. Within the Children and Families Specialty Plan (CFSP), regardless of location, members have access to consistent benefits that include physical health, behavioral health, pharmacy, intellectual/developmental disabilities (I/DD) services, long‑term services and supports, unmet health‑related resource needs, and integrated care management. North Carolina residency is required.
Sign‑on Bonus:
$1,500
Location Harnett and Robeson Counties – candidates must live in or near one of these counties.
Hours & Travel General business hours, Monday through Friday.
Travel within the region is required. When not in the field, the role is performed virtually from home.
Role Summary This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as needed, with occasional office attendance for meetings or training. The Care Extender (Peer Support Specialist‑FC) engages with community partners, provides education, and strengthens relationships to support individuals and families navigating the CFSP Foster Care program.
Primary Duties
Engage in outreach, engagement, and follow‑up with members, caregivers, and families to support care management efforts.
Support care managers in advocating for foster parents and caregivers, addressing their needs or challenges with various agencies and systems.
Coordinate services and appointments, including wellness reminders and arranging transportation.
Assist the care manager in assessing and addressing unmet health‑related needs.
Requirements
High school diploma or equivalent plus at least 2 years of related experience; or any combination of education and experience that provides equivalent background.
Preferred Qualifications
Lived experience navigating similar challenges (must).
Peer Support Certification preferred.
Excellent professionalism and effective communication skills when interacting with individuals, providers, and stakeholders.
Strong written and verbal articulation.
Ability to build respectful relationships with individuals across a wide range of disabilities and health conditions, and with their families.
Strong collaboration within a team environment and ability to follow guidance from the Care Manager and Care Management department.
Compensation Sign‑on bonus: $1,500.
