Job Summary A Hospice Sales Liaison at Kindful Health is an integral part of our team. The Liaison is responsible for establishing and developing new business contacts, maintaining current relationships and developing and implementing a sales and marketing plan to cover the assigned sales/marketing region. The Liaison must be comfortable communicating with patients, families, co‑workers, physicians, social workers, discharge planners and external vendors.
Essential Education and Experience • Bachelor's degree preferred and at least two years of Hospice sales experience. Proven track record of success and a strong “hunter” mentality. • Minimum of 2 years of hospice sales experience with established relationships in the San Antonio market.
Responsibilities & Duties
Generate qualified, professional referrals to achieve hospice admissions to meet short- and long-term target account goals.
Maintain a complete, up-to-date record of referral sources including physicians, hospitals, skilled nursing and senior living facilities’ management, discharge planners and case managers within an assigned territory in the CRM.
Assist the care center in obtaining all required documentation to complete the referral-to-admission process.
Evaluate patient referrals to determine appropriateness for admission to hospice.
Provide consultation and counseling to referrers, patients and families about Kindful's hospice care options.
Communicate all patient health, financial, psycho-social, spiritual, medical & DME needs to the care center and coordinate with the care team to ensure Kindful allocates appropriate resources to meet patient needs.
Act as liaison to patients and families during the transitional period between patient discharge and the initiation of hospice services.
Master the sales model and utilize the sales process daily.
Develop and maintain client relationships within a competitive marketplace.
Establish and promote ongoing collaborative relationships with referral sources, institutional personnel and other facilities to ensure coordinated referrals.
Prepare business plans and maintain target lists; prioritize accounts per the market sales plan.
Respond to customer complaints in a timely manner.
Perform other duties as assigned.
