JOB SUMMARY Under direction of registered nurse, licensed physical therapist, or a licensed occupational therapist provides basic home nursing services to patients.

Work Schedule: This is a Part-Time position, hours of 8-5pm. 20 visits per week with flexible hours.

EXPERIENCE AND EDUCATION

Required

Education High School Diploma or equivalent Experience 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility) and 1 year of full-time experience within the last five (5) years in direct client care in an agency setting. Licenses and Certifications (BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) and (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance and (CNA) CERT NURSE ASSISTANT Nurse Aide certification listed on the HHSC nurse aide registry (NAR). JOB DUTIES

Performs duties as assigned on the home health care plan prepared by a registered nurse. Obtains and documents patient vital signs, including temperature, weight, respiration, and pulse. Reports changes in patient's mental or physical condition or in home situation to nursing supervisor. Assists other members of the nursing team and/or other disciplines. Provides personal care assistance to patients with all Activities of Daily Living (ADLs), light housekeeping, meal preparation, and other specialized duties as needed. Provides assistance to patients with ambulation, safe transfers, and exercises as assigned. Maintains patient records and charts to ensure an accurate record of treatment. Maintains confidentiality of patient data. Assists in maintaining a healthy, safe environment. Attends mandatory in-service educational programs as required by federal and state regulations. Performs other related duties as assigned or requested. SECURITY AND EEO STATEMENT
