As a Banner Home Health Physical Therapist, you will work within a sub‑assigned geographic territory that offers smaller travel distances, a supportive environment, and the choice of pay per visit or hourly, with mileage reimbursement, a company cell phone, and a tablet for documentation in our EMR system (Home Care Home Base).

You will treat patients with a variety of clinical conditions and goals—post‑op orthopedic, neuro‑rehab, cardiac, and more. The position is scheduled Monday–Friday with occasional Saturdays and located in the West Valley POD 7.

Position Summary This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national standards, regulatory agencies, and departmental policies.

Core Functions

Evaluate patient’s functional abilities and limitations, determine intervention needs, and establish a plan of care and treatment goals within the department’s time frame. Identify and prioritize key limiting factors and engage patient and family/caregiver in goal setting and discharge planning.

Implement individualized treatment plans that consider the patient’s physical, social, spiritual, cultural, educational, and age‑specific needs. Provide skilled treatment safely, assess ongoing effectiveness, modify the plan as needed, and involve the patient and family in decision‑making. Educate the interdisciplinary team and the patient about progress and techniques.

Document treatment and assessment in accordance with professional practice guidelines and regulatory requirements. Documentation must be legible, evidence‑based, and completed within established timeframes, reflecting unskilled intervention and patient progress or lack thereof.

Communicate pertinent patient information to ensure optimal follow‑through of care. Liaise with physicians, nurses, patients, family/caregivers, and the interdisciplinary team, and address routine or difficult problems by consulting physicians as needed.

Adapt to assignments related to caseload, scheduling, and staffing shifts, and assist in other patient care areas as required.

Supervise and delegate to PTAs, ATCs, Therapy Technicians, and students in accordance with policy and regulatory requirements. Participate in orientation and ongoing training to ensure personnel competency.

Attend required departmental meetings, participate in process improvement initiatives, and meet productivity standards set by the department.

Interact professionally with all levels of staff, physicians, patients, families, and external contacts such as other health‑care institutions, community agencies, and physician offices to facilitate information exchange and clarify orders.

Minimum Qualifications

Graduated from an APTA accredited school of Physical Therapy or equivalent.

Current Physical Therapy license in the state of practice and BLS certification.

Demonstrated clinical competency, experience, training, and education relevant to the job through interview behaviors.

Proficiency with computer software applications used in an automated patient‑care environment, including document‑capture, scheduling, and electronic medical records.

Knowledge of patient‑care equipment, evaluation tools, therapeutic modalities, and common office equipment (copier, multi‑line telephone, fax, computer, pager).

Employees in Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it throughout employment.

Preferred Qualifications Additional related education and/or experience.
