Job Responsibilities
Read and interpret health record documentation to identify all diagnoses and procedures
Assess the adequacy of health record documentation to ensure it supports all diagnoses and procedures to which codes are assigned
Apply knowledge of anatomy, physiology, clinical disease processes, and coding terminology to assign accurate codes
Utilize Uniform Hospital Discharge Data Set (UHDDS) definitions to select principal diagnoses and procedures
Assist patient billing services to submit clean claims for medical necessity
Qualifications
High School Diploma or GED
Understanding of CPT and ICD-10 coding systems
Certification as CCA, CCS, CPC, RHIT, or RHIA
Minimum of two (2) years of coding experience
Knowledge of disease processes and surgical procedures
Preferred Qualifications
Associate's or Bachelor's degree in Health Information Management
Preferred RHIA or RHIT certification
Additional certifications in health information management or related fields
