Job Summary
The Physical Therapist plays a key role in supporting patients, clients, and students as they work to regain function, reduce pain, and enhance their overall quality of life. In this position, you will conduct comprehensive evaluations, develop individualized treatment plans, and implement evidence-based interventions to improve mobility, independence, and performance of daily activities.
This role offers the opportunity to provide services across a wide range of settings, including outpatient, inpatient, home health, long-term care, school-based, and business environments. In addition to direct patient care, the Physical Therapist contributes to prevention and wellness initiatives through screenings, education, and health-promotion services. Through these efforts, you will help individuals achieve their therapy goals and reach their highest level of functional potential.
Essential Functions
Patient care:
Interpret and implement provider referrals and referrals from staff at contracted schools.
Perform initial evaluations and assessments.
Develop a plan of care based on the initial evaluation that includes therapy interventions to address short-term and long-term goals and outcomes.
Implement a plan of care providing evidence-based physical therapy strategies and interventions.
Monitorpatient/client response to treatment, evaluate progress toward goals, and modify plan of care as indicated.
Establish the discharge plan.
Document according to best practice, federal and state regulations, and insurance requirements.
Delegate tasks to be rendered by a licensed physical therapist assistant or other support personnel.
Report to the Therapy Representative updates related to inpatient, home health and contract facility rehab goals and discharge planning, scheduling, and coordination between disciplines.
Communicate frequency and duration of appointments with scheduling staff.
Educational, administrative, and other service responsibilities:
Report to the Supervisor regarding scheduling, time off, departmental budget, supply and equipment needs, policies and procedures, quality assurance, statistics, and safety.
Conductand participate in patient, student, caregiver, and staff education for therapy, medical, nursing and school personnel, and volunteers in physical therapy techniques and services.
Participate in community service, public relations, public speaking, and advertising activities.
Monitor function of equipment and assure that equipment is kept in proper and safe working order.
Complete continuingeducation to stay current with regulations, best practice techniques and interventions, and practice ethical and evidence-based Physical Therapy according to the American Physical Therapy Association guidelines.

Demonstrate computer skills to operate EMR, electronic exercise programs, and basic Microsoft Office Programs.
Qualifications
Education
Graduate of an accredited professional physical therapy program required.
Licensure: Must have an active Physical Therapist license to practice in the state of Nebraska.
