Primary Responsibilities

Evaluate the patient's functional status and physical therapy needs and assist the physician in developing the therapy plan of care.

Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other health‑care team members.

Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.

Treat patients to relieve pain, restore function, and maintain maximum functioning using acceptable standards of practice.

Instruct patients, families, and the health‑care team about the plan of care, functional limitations, and progress toward goals.

Conduct visits according to the plan of treatment, complete a progress note for each visit, and submit progress notes per LHC policy.

Complete all patient evaluations and develop the PT plan of care within state guidelines.

Report outcomes of evaluation, goals, and projected care frequency.

Required Qualifications

Current Physical Therapy licensure in the state of practice.

Current CPR certification.

Valid driver's license and vehicle insurance, or reliable public transportation.

Ability to provide care in any home situation, irrespective of client characteristics.

The hourly pay for this role ranges from $34.23 to $61.15 based on local labor markets, education, work experience, and certifications.
