Job Responsibilities
Assists in implementing timely disease intervention strategies.
Performs investigations, data collection, analysis and interpretation.
Performs reporting and surveillance activities.
Prepares documentation of all communicable disease case investigations.
Provides case management services (home visits, clinic referrals, patient education).
Provides diagnostic, therapeutic, and educational services in clinic and field settings.
Provides educational programs, seminars and in‑service training to enhance public understanding.
Minimum Qualifications
Associate degree from an accredited college/university and two (2) years of related experience; OR
Four (4) years of experience in a medical‑related field; OR
Licensure as a registered nurse and one (1) year of nursing experience; OR
One (1) year of experience at the lower‑level Communicable Disease Spec 1 (HCP080) or equivalent position.
