At Corus Hospice, you join a team shaped by five generations of care — where trust, collaboration, and courage guide the way we serve. As a Medical Social Worker, you help patients and families navigate some of life’s most vulnerable moments with steadiness, empathy, and clarity.
This role blends autonomy in the field with deep interdisciplinary teamwork. You will support patient comfort, emotional well-being, and safe transitions while helping families feel seen, understood, and grounded throughout the hospice journey.
Key Responsibilities
Provide social work services under a physician’s plan of care, supporting patient and family emotional, social, and practical needs
Help the interdisciplinary team understand family systems, psychosocial dynamics, and environmental factors influencing care
Connect patients and families with community resources, financial supports, and social services
Participate in care planning, case conferences, and safe discharge or transition planning
Document assessments, visits, interventions, and coordination in a timely and compliant manner
Maintain relationships with community organizations to support patient needs
Uphold Medicare CoPs, state regulations, and professional social work standards
Requirements
Master’s degree in Social Work (MSW) from a CSWE-accredited program
One year of social work experience (healthcare preferred; hospice experience not required)
Strong communication and relationship-building skills
Deep empathy and a steady presence in emotionally intense situations
Skill in navigating complex family dynamics with calm, clarity, and respect
Knowledge of community resources and psychosocial assessment
Valid driver’s license, reliable transportation, and required auto insurance
