Job Summary As an Elder Care Specialist, you serve as a cornerstone of a close‑knit, elder‑directed household. This role blends hands‑on caregiving, home‑keeping, and life enrichment in a relational, empowered team model. You must hold an active CNA license. Eligible to take on expanded responsibilities—including mentoring new Elder Care Trainees and working toward the role of Elder Care Coordinator, a leadership position supporting the functioning and rhythm of the home.

Essential Job Functions

Relational Care & Clinical Support

Deliver high‑quality, person‑directed care to Elders in accordance with individualized care plans and CMS standards.

Assist with all activities of daily living (ADLs), including bathing, dressing, eating, toileting, and mobility.

Monitor and report changes in condition; support documentation necessary for Medicaid and Medicare billing compliance.

Collaborate with nurses and clinical support team to ensure clinical needs are met in alignment with Elder preferences.

Household Operations

Take shared ownership of daily household tasks including meal planning and preparation, housekeeping, and laundry, ensuring the home is clean, comfortable, and welcoming.

Use culinary skills to prepare nutritious, home‑style meals in an open kitchen, fostering connection and dignity in dining.

Participate in inventory, supply ordering, and maintenance of household routines.

Meaningful Engagement & Community Life

Design and implement activities that bring purpose, enjoyment, and social connection to Elders’ lives.

Help cultivate a home culture of inclusion, creativity, and joy.

Support transportation and accompaniment to appointments, social events, or community outings.

Leadership Development & Mentorship

Serve as a role model and mentor for Elder Care Trainees and peers, supporting their growth in caregiving, home‑keeping, and Sheridan Green House values.

Participate in onboarding, peer coaching, and skill‑building for team members.

Work collaboratively with Leadership and clinical support team to strengthen team self‑management and shared decision‑making.

As skills grow, take on project leadership or coordination tasks in the areas of scheduling, enrichment, or household organization.

Pathway to Elder Care Coordinator

Eligible to progress into the Elder Care Coordinator role upon demonstrating:

Mastery of Sheridan Green House core practices and peer leadership.

Strong communication, organization, and coordination skills.

Initiative in mentoring, team development, and quality improvement.

Receive additional training and mentorship to support leadership in household operations, team dynamics, and regulatory compliance.

Practice and demonstrate effective coaching strategies to support team and individual development.

Support for Clinical Services & Reimbursable Care

Under the supervision of licensed staff, assist in tasks that support Medicaid and Medicare billing requirements, including:

Promptly reporting observations related to changes in health status or behaviors.

Participating in documentation, data collection, or charting as trained and allowed by regulation.

Supporting restorative care activities and participation tracking as part of care plans.

Ensure care tasks align with state scope‑of‑practice laws for non‑certified staff and Sheridan Green House policies.

Assist in fulfilling elements of CMS documentation standards necessary for accurate reimbursement and quality reporting (e.g., MDS inputs, care plan updates).

Position Qualifications

Active Wyoming CNA license and listed in good standing on the Nurse Aide Registry.

Completion of Sheridan Green House Elder Care associate/trainee training or equivalent experience in elder‑directed care preferred.

Strong interpersonal skills and demonstrated commitment to relationship‑based care.

Ability to work collaboratively in a self‑managed team environment.

Experience in cooking, cleaning, or life enrichment strongly valued.

Desire to grow into leadership roles within the Green House Model.

BLS required.

Central Registry required.

Additional Skills

Able to communicate effectively in English, both verbally and in writing.

Additional languages desirable.

Basic computer knowledge.

Demonstrated effective organizational skills.

Specific demands not listed: Possible exposure to blood and/or body fluids, infectious disease, hazardous waste requiring the use of personal protective equipment. Exposure to odorous chemicals, specimens, and latex products.
