Position Summary The Life Enrichment (Care) Coordinator is primarily responsible for supporting, training, and directing in-home attendant care providers while ensuring services remain aligned with Medicaid and HCBW requirements and Golden Years’ values-driven culture. This role oversees the scheduling board to ensure all authorized hours are correctly scheduled and worked, verify that EVV (Electronic Visit Verification) signatures are completed, and manages incident reports and follow-up for in-home services. The coordinator serves as a leadership and quality anchor between the Edmonton office, in-home care providers, participants, and families.
Key Responsibilities
Support, train, and coach in-home attendant care providers on duties, participant rights, safety standards, and Medicaid/HCBW expectations.
Model and reinforce Golden Years’ culture of respect, kindness, empathy, and community enrichment in all interactions.
Manage the schedule board for in-home attendant care, ensuring all authorized hours are properly scheduled, covered, and reconciled.
Monitor EVV data to confirm visit accuracy and signatures; address missing or incorrect entries promptly.
Track utilization of authorized hours, identify gaps or overages, and work with leadership to maintain compliance and continuity of care.
Receive, document, and coordinate incident reports related to in-home services, ensuring prompt follow‑up and communication.
Collaborate with case management and leadership to align in-home tasks with person-centered plans of care and safety plans.
Participate in care team meetings, quality assurance reviews, and internal audits related to waiver services, EVV, and incident management.
Maintain correct, prompt documentation and reports to support audit readiness and regulatory compliance.
Provide occasional support to center-based life enrichment or care coordination activities as needed.
Schedule
Monday through Friday
1 Saturday per month required (for coverage, training, events, or program needs)
Office-based position at 903 W Stockton Street, Edmonton, KY 42129
Compensation, Training & Performance Structure
Starting pay band: $18.00 - $22.00 per hour, based on experience, leadership skills, certifications, and overall qualifications.
New team members receive 46 weeks of training with experienced staff, plus weekly feedback sessions to track retention, knowledge, application, and performance.
All hires begin with a 90‑day probationary period.
Formal performance review at 6 months of employment.
Ongoing performance and development include:
- Monthly 1:1 check‑ins.
- Quarterly 4x4 meetings (4 key goals, 4 key focus areas).
- Semi‑annual 360 performance and development reviews.
Ongoing education and development provided by the Golden Years care team.
Qualifications & Skills
Experience in home and community‑based services, attendant care coordination, case management, or similar role; Medicaid/HCBW experience strongly preferred.
Demonstrated leadership skills in coaching, training, and supporting direct care staff.
Familiarity with scheduling systems and EVV platforms in a healthcare/HCBS setting.
Strong organizational skills with the ability to manage multiple schedules and documentation.
Clear, professional communication skills with staff, participants, families, and interdisciplinary teams.
High level of reliability, integrity, and discretion with sensitive information.
CPR, First Aid, and other relevant certifications preferred (or willingness to obtain within a defined period).
Disqualifiers & Pre‑Employment Requirements
Must be able to successfully pass all required pre‑employment checks, including:
- Criminal background check.
- Drug screen.
- TB test.
Must have a valid driver’s license and current proof of auto insurance.
Candidates who are unable to meet these requirements will not be eligible for hire.
Offer & Onboarding Expectations
Once a formal offer is presented, candidates will have 48 hours to accept the offer and complete all required onboarding tasks.
Failure to accept within 48 hours or to complete onboarding tasks within the designated period may result in the offer being withdrawn.
