Peer Support Specialist White House Clinics – Berea Primary Care Clinic Work schedule: 11:00 AM – 7:00 PM, Monday through Friday with occasional evenings and weekends.

Core Responsibilities

Meet with patients during medical and/or behavioral health visits to facilitate engagement in substance‑use‑disorder (SUD) treatment services and collaborate with care teams.

Develop patient‑centric care plans and enhance motivation through motivational enhancement strategies and personal recovery storytelling.

Offer referrals and connect patients to alternative levels of care when needed, and schedule SUD services at White House Clinics.

Maintain telephone contact with patients awaiting services, reminder patients of appointments, pill counts, random urine drug screens, and help identify barriers to care.

Work with patients to develop a Person‑Centered Recovery Plan, establishing recovery goals and resources for success.

Provide 24‑hour telephone recovery support to patients at risk of relapse, including weekly contacts and updates for care teams.

Identify and share community recovery support meetings (e.g., AA/NA, Celebrate Recovery) and, when appropriate, transport patients to meetings.

Serve as liaison between the treatment team and patients, documenting all contacts in the electronic health record.

Meet twice a month with a supervisor for required supervision per state regulations; maintain a supervision history log.

Job Requirements

High school diploma or GED.

Kentucky Certified Peer Support Specialist license.

Valid driver’s license and reliable personal vehicle.

Commitment to meeting supervision and continuing education requirements per 908 KAR 2:220; supervision provided by an organizational LCSW.

Qualifications

At least two years of stable recovery in the community.

Commitment to helping the SUD population, consistent with the MAT services offered by White House Clinics.

Professional, warm demeanor that fosters collaborative relationships with patients, providers, and external resources.

Strong problem‑solving skills, initiative, and ability to work independently.

Ability to maintain healthy boundaries and adhere to ethical guidelines for the PSS role.

Salary & Benefits

Starting salary: $15.04 per hour.

Employment type: Full‑time, 37.5 hours per week.

Candidates should anticipate work outside normal business hours and participate in an on‑call rotation that may include evenings, nights, and weekends.

Work location: Office environment and community settings (e.g., emergency department, jail).
