SCAN Group is a not‑for‑profit organization dedicated to keeping older adults healthy and independent across California, Arizona, Nevada, Texas and New Mexico. The Peer Advocate Bilingual role supports members at risk for specific health conditions, providing education, motivation and care coordination while celebrating the SCAN community.
Peer Advocate Bilingual Required Must be available to work onsite storefront Henderson, NV. Must be a current Scan Health Plan member.
Responsibilities Make outbound calls to SCAN members based on identified health initiatives. Build rapport with members by sharing personal experiences as a SCAN member and peer. Coach members to create and implement strategies that minimize risks associated with common health conditions. Encourage and assist members who may encounter obstacles to seeking care. Document all required information in the care management system during phone calls. Maintain client confidence by keeping information confidential. Utilize department job aids and training materials. Support the achievement of SCAN’s Vision and Goals. Contribute to team effort by accomplishing related results as needed. Other duties as assigned.
Qualifications High School Diploma or equivalent; preferred. Current SCAN Membership, required. Proficient with operating a computer, including online timesheets and call center software. Excellent communication skills, including the ability to articulate personal membership experience. Strong interpersonal skills and written and verbal communication skills. Ability to work independently as well as part of a team. Strong organizational skills. Ability to work in a fast, loud and changing environment.
