About the Position – Re‑Entry Specialist The
Re‑Entry Specialist
provides support for adults navigating recovery and re‑entry after incarceration. Working closely with clinical and peer teams, this role assesses needs, develops transition plans, and coordinates care. The focus is on helping members build independence, achieve stability, and lead purposeful, self‑sufficient lives.
Your Impact Will Include
Delivers high‑quality case management to support members' recovery and re‑entry goals
Effectively communicates and coordinates care with other treatment team members
Partners with members for goal attainment with employment, independent living, community involvement, and building meaningful social connections
Connects members with needed resources and helps facilitate access to employment, housing, and other support programs
Acts as an advocate for members with respect to insurance, financial assistance, and service accessibility
Helps support and coordinate smooth transitioning and discharge planning upon program or treatment completion
In addition, all other duties as assigned
Where You’ll Work
Primarily onsite at our offices and within correctional facilities.
Traveling to various institutions to meet directly with participants.
Using standard office equipment, plus plenty of face‑to‑face interaction.
Some days will require long periods of sitting, standing, or walking.

Qualifications Required Skills:
Ability to sit for prolonged periods – Intermediate
Behavioral Health Experience – Intermediate
Lived Experience – Intermediate
Peer Coach Experience – Intermediate
Education
High School or better.
HS Equivalency or better.
Experience
Excellent communication, organizational, time management, and critical thinking and problem‑solving skills required.
Effectively demonstrates versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.
Effectively collaborates and works within a team, while also effectively demonstrating the ability to take initiative and work well individually.
Peer and Recovery Support Specialist (PRSS) Certification is required for provision of direct PRSS services.
2 years Behavioral Health Technician designation required (2+ years behavioral health work experience and/or higher education).
Case Management and/or BHT Certification and/or expertise is preferred.
