POSITION SUMMARY The primary purpose of your job is to return patients to optimal physical function and facilitate achievement of patient, physician, and therapy goals. You will work primarily with patients in the memory care unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community’s administration as well as Policies and Procedures.

Education / Qualifications

Employee must hold an Associate’s Degree from an accredited college.

Must hold a current and valid license granted from the state of employment.

Current and valid OTA license granted from state of employment.

Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the community.

Must have minimum of one (1) year of experience in long-term care and with adults with cognitive decline.

Shift & Wage

Full-Time Opportunity Available

Shift Needed: Varies

Sign-on Bonus: $5,000
