is looking for a Specialist Outreach Support to work with providers and managed care plan members to improve quality outcomes.
Responsibilities
Provide liaison services between providers and managed care plan members by identifying strategies to address gaps in service and care.
Implement and monitor all phases of programs, including in‑service trainings, assessment tools, performance measures, outcomes, and goals.
Act as a facilitator and consultant to enhance education goals in caring for hospital customers (in‑patient, outpatient, and community).
Coordinate instructional activities of multi‑disciplinary team members and comply with physician requests for patient instruction.
Prioritize tasks and demonstrate sound time‑management skills to fulfill program objectives.
Evaluate plans and track outcomes, develop plans as needed, and build relationships with community‑based organizations and healthcare providers.
Requirements Required
High School Graduate or GED with two (2) years related work experience.
Preferred
Bachelor’s degree in Human Services, Social Services, or related field.
Where You’ll Work Dignity Health Connected Living has been serving seniors in Shasta County since 1968. Our programs are designed for seniors and people with disabilities and are delivered in a compassionate environment.
Compensation $24.00 - $25.00 per hour.
