Intake Specialist – Alive Hospice
Location: Alive main campus, Nashville, TN
Status: Part Time
Hours: 24 Hours/week

Summary Under the direction of the Director of Intake & Admissions, the Intake Specialist responds to inquiry and intake calls, assists with administrative duties and is cross-trained for other provider relations roles. The Intake Specialist supports the admission of patients, communication with providers (internal and external), data entry and documentation.
Essential Duties And Responsibilities
Represents the agency as assigned to promote services and co‑markets with provider relations representatives.
Serves as back‑up to Director of Intake & Admissions as needed.
Serves as liaison and provides feedback from referral sources.
Collaborates with physicians, social workers, discharge planners, and clinical coordinators to gather all available information for treatment plans (discharge orders, etc.).
Answers and responds to multiple calls including inquiry, intake, family and healthcare providers.
Gives accurate, consistent, compassionate attention to each call.
Maintains patient, family and staff confidentiality.
Files, faxes, logs and maintains data in an accurate and readily accessible fashion.
Enters data, manipulates and retrieves data in proper reporting structure.
Routes paperwork appropriately and in a timely manner.
Assists with administrative duties as assigned by Director of Intake & Admissions / Director of Clinical Partnerships.
Other duties may also be assigned.
Requirements Education and/or Experience Associates Degree (A.A.) from college or technical school; one year’s related experience and/or training; or equivalent combination of education and experience.
Certificates, Licenses, Registrations Current driver’s license and automobile insurance as required by Tennessee State Law.
