Base pay range
$24.50/hr - $24.50/hr Goldie Floberg QIDP's do not work as DSPs or Supervise DSPs. On-call hours are not covering DSP hours, only medical emergencies. Position Summary: This position works with the people served to help them create a life of their choosing and to help them advance toward achievement of their personal goals and desired outcomes. Under the direction of the assigned QIDP, this position will assist with facilitating the coordination of all program services that affect the life of each individual while ensuring case regulatory compliance. Schedule: This is a full‑time position that requires a staff member to work a minimum of 80 hours per two‑week pay period. The Case Manager works primarily during office hours (8:30 a.m. – 5:00 p.m.) with flex scheduling, refer to policy 502 Work Schedules for details. Operational needs will require working more than 80 hours per pay period at times and working outside of office hours at times. The Case Manager spends a significant component of their time working hours in the field directly with individuals and Direct Support Professionals. Must meetall ofthe requirements and be able to performall ofthe essential job functions of the DSP position. Must complete and maintain all DSP training requirements including annual re‑training and medication administration privileges. Must take and successfully pass the S-TOFHLA reading comprehension test. Demonstrate sufficient communication and writing skills to complete required documentation. Must successfully pass all required background checks and not be convicted of any crime that violates DHS or DCFS requirements for employment with Goldie Floberg. Must be able and willing to work with individuals who exhibit contextually inappropriate behavior, including, but not limited to, physical aggression towards staff and/or people served, self‑injurious behavior, and verbal aggression across a variety of environments including home and community settings. Must be able and willing to work with individuals who need significant or total physical care for activities of daily living including, but not limited to, bathing/showering, toileting (including wiping), eating, and dressing across a variety of environments including home and community settings. Must be able to complete data entry using a laptop,computeror tablet. Ability to work effectively as part of a team. Must have memorized our mission and values. Essential Job Functions: Licensing Conduct regular, in‑person home inspections to ensure that people served are safe andhappyand that homes meet licensing requirements for safety, cleanliness, quality, and person‑centeredness. The frequency of inspections will be set by the Chief Operating Officer but will be no less than monthly for each home. Work with Program Supervisors to ensure that coaching is completed for performance discrepancies discovered during inspections. Work with the Operations Coordinator to resolve any home maintenance needs promptly. Provide DHS DDD, DCFS, and other licensing and regulatorystaffrequested information as directed by the Chief Operating Officer. Participate in licensing and quality inspections when they are conducted by regulators. This will include preparing homes and files, providing tours, and assisting regulators in accessing files and documents. Provide regular updates on people served to guardians as requested.The updates should be in the form of an email or other format that provides evidence of the updates. Work with assigned QIDPs to schedule and conduct a community support Team (CST) meeting for a person-served and their guardian when requested and when there is a significant life change. Life changes include a change in placement (including initial intake), day services or school setting, the loss of a loved one, moving to a different home, a change in housemates, significant injuries, incidents involving mistreatment, behavioral incidents that involve hospitalization or police involvement. Maintain professional and frequent communication and appropriate information sharing with stakeholders such as school staff, day services staff, employers, and other stakeholders and service providers involved with the person served. Work with people served and their guardian to address any concerns that they have in a prompt manner.Bring other staff into the problem‑solving process as needed. Document the outcome of all related meetings with a summary email to applicable stakeholders. Notify DSP staff, Program Supervisors, and Nursing when home visits will occur. Service Continuity (When a Compliance QIDP is not employed) Ensure that people served are connected to and have uninterrupted access to financialsupports(social security benefits, supplemental security benefits, LINK, Medicaid, etc.) Assist with initial intake and annual risk/support assessments, as needed. Behavioral Supports Conduct a visual spot check of BER graphs after being notified of a behavioral incident. Work with program Supervisors to ensure coaching is completed for any performance discrepancies related to BERs not being completed. Notify the appropriate RN when a medicalwork up and examinationis needed for behavioral reasons. Schedule behavioral consults, using the applicable form and process as needed but at least annually for each person served with a BSP. Ensure that staff who know the person served well and assigned supervisor(s) are participating in the consultation. Review Behavior Support Plans and ensure follow‑up on any environmental safety measures or modifications and/or materials and supplies ordered by the behavior staff within the requested time frames. Participate in Psychiatric appointments for individuals on their caseload. Assist with coordinating guardian attendance, as desired by the guardian. Assist QIDP to complete SST referral application.Participate inintake and routine SST meetings as well as coordinate with DSP staff when ICPN staffwill beat the site for observation. Home and Community Based Services (HCBS) Oversee efforts to ensure that the agency isup‑to‑datewith Home and Community Based Services (HCBS) waiver requirements and best practices, primarily in the 37U setting. This will require collaboration with multiple agency teams/positions as well as attending routine training, meetings/discussions regarding HCBS waiver requirements. Key areas of oversight and responsibility include, but are not limited to, the following:
Personalized activity planning Communication supports Home/bedroom accessibility, privacy, and personalization Menu planning Assessment and implementation of physical modifications or supports that assist individuals with independence at home and within the community Household contribution
Attend routine training, meetings/discussions regarding HCBS waiver requirements. Conduct data collection compliance reviews monthly of HCBS data such as community activities and visitor logs. Complete applications for individuals to CDS and complete DRS process Teach the person-served human rights and sex education programs to the people we serve at least once a year, more if necessary. Complete Supports and Risk assessments annually with individualsto submit withPersonal Plan materials by designated deadlines. Registering individuals and coordinating personalized activities with (RPD/YMCA/Camp). Create monthly activity calendars with community involvement options for sites. Register individuals for school annually and follow up, with guardians to obtain all required school and medical consents. Assist with monitoring goal tracking, follow up, and coaching as needed to ensure individual goals are being run and documented. Other Responsibilities: Keep an electronic telephone log of each phone call made and received while on duty. List date and time of phone call, who called or who you called and a summary of what the conversation was about. Phone numbers need to be recorded for all outgoing calls.
