Sign-on Bonus: $2,000
Our nursing staff provide care to adults with severe and persistent mental illness in and out of the office. The nurse provides health assessments, monitors vitals, administers medications, and works in coordination with the team of Case Managers, Therapists, and Nurse Practitioners/ Psychiatrists to help patients with recovery and wellness goals. This person also maintains compliance with all standards to assure high quality care.
Key Responsibilities
Conduct health assessments for patients with severe and persistent mental illness, addressing both physical and mental health needs.
Monitor and record patients' vital signs and report significant changes to the healthcare team.
Administer prescribed medications, following all guidelines for accuracy and safety.
Collaborate with Case Managers, Therapists, Nurse Practitioners, and Psychiatrists to develop and implement individualized recovery and wellness plans for patients.
Maintain accurate, up-to-date medical records, ensuring compliance with organizational and regulatory standards.
Provide patient education regarding medication management, wellness strategies, and healthy lifestyle choices.
Ensure compliance with all standards of care, including clinical, ethical, and regulatory guidelines, to guarantee the highest quality of care.
Participate in ongoing professional development and team meetings to enhance service delivery.
Hours Monday-Friday: 8 am to 5 pm – No evenings! No weekends! No holidays!
Qualifications
Ohio RN/LPN license.
Must be willing/able to provide some community-based nursing services and transport clients in your personal vehicle.
