Overview:
Bring your passion and expertise to our Home Health program. Our Social Workers are the cornerstone of patient-centered care, supporting clients and families in crisis and connecting them to community resources.
What You’ll Do:
Guide and counsel individuals and families in crisis, ranging from unemployed to those needing foster homes or elderly care.
Assist clients in obtaining government funds, education, or treatment.
Secure information—for example, medical, psychological, and social factors—and evaluate client capacities.
Help patients and families through conferences regarding needs and plans, mobilizing inner capacities and environmental resources.
Help clients modify attitudes and behavior patterns to improve social functioning.
Develop a service plan to restore clients to optimum social and health adjustment.

Qualifications:
Master’s Degree in Social Work
Five or more years of clinical social work experience
Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) license
Ability to perform psycho-social assessments and develop and implement viable care plans
