The Center for Human Development, (CHD) is seeking Senior Direct Care Staff to work within our residential group homes in the Springfield, MA and surrounding towns of Western Mass. This is a full time opportunity: The shift is 3:00 PM - 11:00 PM, with scheduling options of Tuesday-Saturday or Sunday-Thursday, depending on the specific needs of the program. Candidates must hold active MAP certification to be considered for this role. Your role as a Senior Direct Care Staff: The Senior Direct Care Staff plays a crucial role in managing medication administration, appointment preparation, and scheduling adjustments during shifts, particularly from 3 pm to 11 pm. This role includes ensuring compliance with agency standards, fostering a clean and welcoming home environment, and engaging individuals in community activities. Working collaboratively with the agencys team model, supporting client growth and independence, and ensuring safe, consistent care for those served. Some additional duties of this specialized role include, but are not limited to: Conduct of weekly Mini MAP audits. Manages medication administration according to MAP regulations. Prepares clients for appointments, completing paperwork and assigning attending staff to ensure clients feel supported and prepared. Actively engages clients, assisting with daily living activities such as personal hygiene, meal prep, housekeeping, and laundry. Promotes client participation in community events, coordinating outings and social gatherings to foster social connections and community involvement. Transports clients to and from appointments and activities. Verifies that staff training certifications are current in Relias, monitors training completion, and assists with on-the-job guidance for new staff as directed by the Supervisor. Manages shift coverage from 3 pm to 11 pm, handling call-outs, and ensuring consistent staffing and medication administration across all shifts. Reviews shift notes in Netsmart, ensuring completion of responsibilities and accuracy in documentation for continuity of care. Reviews and updates client records to ensure comprehensive documentation of services, medical records, ISP, and PMPT data. Monitors the completion of incident reports and accurately documents incidents, following agency protocols. Assists with managing timesheets, schedules, and incident documentation to maintain organized and accurate record. Conducts routine checks of the site vehicle using a checklist, reporting any issues or maintenance needs to the Supervisor. Requirements: Must hold active MAP certification. All candidates must complete and pass background record screening process. Must have a vehicle for work use and current drivers license. Associate degree in human services from an accredited institution is preferred. Current First Aid, CPR certification required. Minimum two years of experience with developmentally disabled adults. Previous supervisory experience preferred. This position starts at $20/hour, with the opportunity to increase to $23/hour following a successful 90-day evaluation.
