Under the supervision of the Senior Program Manager, the Linkage Specialist will identify and link HIV-positive clients to HIV medical care through the implementation of outreach, brief intervention sessions, graduated disengagement, and follow-up. This position will provide high-quality health-related information, referrals, and strengths-based services in a non-judgmental, stigma-free way. Experience working with diverse ethnic and cultural communities is a must, including but not limited to the following populations: people of color, gay/lesbian/bisexual/transgender/questioning community, monolingual and/or undocumented immigrants, substance users, and homeless persons, of varying ages.

Identify and link HIV-positive clients to HIV medical care through the implementation of outreach and brief intervention sessions per contract guidelines.

Conduct patient follow-ups to ensure services were obtained and provide additional support as needed.

Report on intervention activities and propose strategies to address challenges in implementation and emerging trends.

Provide necessary programmatic documentation, including the timely completion of reports, administration of evaluation tools, and other program-related assignments.

Assist in the coordination and participation in community events, health fairs, and festivals.

Other projects as assigned to meet program goals and objectives.

Provide information that demonstrates respect, active listening skills, empathy, and patience in a non-judgmental, stigma-free way.

Provide crisis and short-term intervention for individuals as needed; ensure that clients requiring immediate crisis counseling receive appropriate linkage to licensed professionals.

Refer and actively link clients to medical and essential support services and provide direct practical support, including attending healthcare appointments for newly diagnosed clients and monitoring progress toward care plan goals.

Document contacts and services; maintain files and data for reporting purposes.

Serves as an advocate for clients within the community.

Performs other duties as assigned.

Position Requirements

This position requires a minimum of High School Diploma or Equivalent and a minimum of 2 years’ job-related experience.

BA/BS preferred but not required.

Minimum of 2 years’ experience providing HIV or related case management services to clients living with HIV or at risk for HIV.

Ability to work with a high degree of independence in a fast-paced environment.

This position will require flexible hours, including occasional evenings and weekends.
