Clinical Intervention Specialist - Residential Services Description

Scope of Position The Critical Care Team is seeking a Licensed Clinical Intervention Specialist who will provide consultation, training, and clinical support to outreach professionals while delivering direct interventions as part of a multidisciplinary team. This role focuses on promoting the safety, stability, and well‑being of older adults and medically fragile individuals who have experienced or are currently experiencing homelessness, ensuring that comprehensive, trauma‑informed, and person‑centered care is integrated into all aspects of service delivery.

Essential Duties and Responsibilities

Provide consultation to outreach team staff across NYC, recommending and training on specialized interventions such as conducting relevant assessments; making referrals to community‑based services (e.g., homecare and socialization programs); completing housing applications; advocating for benefits and entitlements; coordinating care and services; and assisting with community navigation, including accompaniment to behavioral health and medical appointments.

Facilitate and lead case‑specific conferences in community settings, including inpatient and outpatient behavioral health facilities, community‑based social service provider sites, participants’ homes, shelters, and locations where unhoused individuals reside.

Train staff and provide direct skill‑building support to promote optimal mental and emotional health, including the use of preventive strategies to reduce medical risk.

Engage individuals regarding substance use, mental health, medical concerns, family and social relationships, stress and symptom management, and medication management.

Actively participate in multidisciplinary meetings.

Travel throughout Manhattan, Brooklyn, the Bronx, and Queens as required.

Qualifications

Master’s degree in Social Work, Mental Health Counseling, or a related field.

Licensed Master Social Worker (LMSW), Licensed Mental Health Counselor (LMHC), or eligibility for a Limited Permit preferred.

Ability to provide consultation services to citywide street outreach teams serving older adults and individuals with chronic medical conditions who are unsheltered or have recently transitioned from street homelessness to housing.

Experience working with older adults, individuals with chronic medical conditions, people experiencing homelessness, and/or in related fields.
