Responsibilities Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Supervise other home care staff as necessary.
Qualifications Must possess an active RN license in both VT and NH. ASN required, BSN or MSN preferred. Nights, Weekend, Holiday and on-call rotation required.
Education Associates or better in Nursing. BSN or higher preferred.
