Responsibilities Essential Functions

Provides a positive environment for individuals served that allows for learning and enjoyment of activities.

Provides consistent meaningful activities.

Provides direct support and training to individuals served.

Encourages individuals to be as independent as possible in their daily routine and takes advantage of every opportunity to teach.

Initiates activities and encourages individuals to participate in the activities.

Performs activities with individuals in the areas of house cleaning, household maintenance, grocery shopping, and nutritious meal preparation and clean up, personal grooming and bathing, toileting, dressing, etc.

Implements goals and objectives as identified in Individual Support Plans and Behavior Management Plans.

Actively listens to individuals and encourages them to express their personal preferences.

Identifies the personal interests of individuals.

Assists with positioning, transferring and personal care of individuals. Must be able to assist up to 50 lbs.

Provides for the dignity of individuals by assuring personal cleanliness including monitoring the condition of personal possessions, clothing and adaptive equipment.

Properly administers medications and promotes a healthy environment.

Provides support for individuals as they access community resources and recreational activities.

Assists individuals with money management and makes application for benefits for individuals as directed.

Transports individuals in agency vehicles or personally owned vehicles for access to community resources and activities such as church, dances, etc. Participates in such activities with the individuals.

Performs safety drills as directed.

Effectively communicates progress and issues regarding individuals served to the Program Coordinator and with other team members as needed.

Adheres to agency policy and procedure in the performance of duties.

Adheres to regulatory and accreditation guidelines in the delivery of services.

Completes agency reporting in accordance with agency protocol.

Completes service summaries, data sheets and all other documentation for each person served.

Completes routine operational paperwork in a timely manner including e‑time adjustment sheets, leave requests, etc.

General Responsibilities

Responsible for the health and safety of individuals served.

Expected to serve as role model for individuals served.

Expected to serve as an advocate and to protect the human rights of individuals served.

Expected to participate in training as required by your supervisor or for all LOGAN staff.

Expected to demonstrate good judgement.

Other duties as assigned.

Qualifications Education

High school diploma or equivalent.

Experience

One year working with individuals with developmental disabilities preferred.

Job Knowledge, Skills and Abilities

For positions with driving responsibilities, you must have a valid driver’s license, vehicle insurance, vehicle registration, and an acceptable driving record that meets the risk management standards for insurability on LOGAN's commercial auto insurance policy.

Ability to communicate well orally and in writing.

Ability to travel within the county and, occasionally, outside the county area.

Contact Responsibility Fulfilling the responsibilities of this job may result in contact with body fluids. LOGAN will provide training and offer Hepatitis B vaccination at no cost to the employee upon request.
