A Brief Job Description: GENERAL SUMMARY
The Outreach Coordinator provides supportive, concrete and case management services to adolescents and women of reproductive years and their families in an effort to improve their health, personal wellbeing and parenting skills. In collaboration with a multi-disciplinary team, the Outreach Coordinator will assess the family’s needs with a focus on providing services directed towards improving safety, preventive health status, self-sufficiency and connection to community resources.
ESSENTIAL FUNCTIONS
Complete comprehensive assessment utilizing interviewing, structured assessment tools and observational skills to support families in identifying their needs.
Document all interventions within required time frame in designated databases.
Complete regular home visits with assigned families and directly transport program participants to medical, emergency social service appointments, or to other locations that are part of the participant’s service goals.
In conjunction with the family, assess, develop and implement family’s goals and objectives for treatment plan and regularly review goal progress.
Assist the families in acquiring tangible needs (housing, financial entitlements, quality childcare, food, essential baby supplies, etc).
Collaborate with Mt. Hope team members, and attend regular staff meetings, ongoing trainings, staff development and any other meetings as needed. Other duties as assigned.
MINIMUM EDUCATION & EXPERIENCE
At minimum, the Health Project Counselor must have a high school diploma. Required
AS degree preferred.
Candidate must have deep roots or shared life experiences within the Rochester community. Required
Experience in the delivery of home-based case management services to an urban, underserved population required. Required
Demonstration of empathy and acceptance of a variety of clients, and the ability to work one-to-one in a supportive, nurturing relationship must be demonstrated. Required
The candidate must demonstrate sound critical judgment in recognizing high-risk situations and respond, based on training and education, in ways that minimize risk. Required
They must have a proven track record of successful engagement and retention of participants and extensive knowledge of community resources. Required
Ability to work independently and within collaborative teams. Required
KNOWLEDGE, SKILLS AND ABILITIES
Must demonstrate the ability to communicate orally and in writing.
Basic computer literacy required.
Basic Microsoft Word Skills as evidenced by the ability to produce basic business letters, check spelling, and sort, save, open and organize electronic files. Required
Ability to learn multiple database programs required.
Willingness to work within the community and to transport clients is required.
Must have a registered automobile and requisite insurance as well as a current driving record that reflects UR safer driver guidelines in order to transport clients. Required
Spanish fluency is desirable but not required
LICENSES AND CERTIFICATIONS
NYS Valid Driver’s License Required
Compensation Range
$17.00 - $22.10/hr.
Employment Full Time, 40 hours per week, Monday through Friday
