Job Details Full-Time or Part-Time | Flexible hours (On-site or hybrid schedule) | Settings: Home Health/SNF/TCU | Home Base: Hutchinson, MN (within a 20-mile radius) | Pay: $39-$53 per hour (based on experience and interview).

Qualifications
Current license as a Physical Therapist or ability to obtain one in the state of practice.
Minimum one year of experience in a health care setting; previous home health experience preferred.
Must meet Clinical Competency requirements.
Ability to travel to patient homes within the designated geographic area.
Valid driver’s license and proof of auto insurance.
Current CPR certification.
Responsibilities
Provide personalized care to patients in their homes, improving independence and quality of life.
Collaborate with interdisciplinary teams and clients’ families to design care plans.
Document and track patient progress and outcomes.
Maintain professional licensing and certifications.
