Supports Coordinator
Join our team at Tri-Valley Care Inc./Merakey. We are seeking a Supports Coordinator to serve individuals in Susquehanna County, Lackawanna County, and/or surrounding counties in Pennsylvania.
Salary & Employment Fee for Service role. Earn: $23.50/hour
Responsibilities
Manage a caseload in the specified counties.
Assist individuals in maximizing their everyday life by identifying needs, strengths, lifetime goals, and service preferences.
Develop individual service plans based on assessments aligned with personal vision.
Maintain regular communication and contact to ensure all necessary supports are intact.
Collaborate with individuals, families, and providers under Positive Approaches and Self-Determination principles.
Provide flexibility for evenings and weekends as required.
