Reports To: Program Director
Trainings: Annual, CPI, HSFAC required within 90 days from date of hire or transfer.
Position Summary Respond to crisis in community within 24 hours. De‑escalate crisis and stabilize child and family unit. This position provides assessment, treatment planning and comprehensive case management.
Duties and Responsibilities
Respond on site to the 1‑800 state triaged crisis call within one hour.
Utilize the DHS assessment tool for identification of risk.
Provide crisis stabilization services geared toward preserving the community placement up to a maximum 72‑hour period, if assessment indicates no immediate danger to self and/or others.
Work with the screening center to support on‑site (community‑based) screening, when indicated.
Coordinate the inpatient admission, if indicated and according to established affiliation agreement.
Link the child/family to community‑based services, when indicated and crisis stabilization is effected, according to established agreements.
Develop the Individual Crisis Plan for a period not to exceed eight weeks if further stabilization is indicated.
Provide relevant feedback to the program coordinator regarding provider network access and response to service authorization.
Document all services according to established standards.
Provide timely relevant/required data to the Project Director as directed.
Documentation must be completed within one (1) calendar day of service delivery.
Minimal Qualifications
Master's Degree in a behavioral health or related human services field.
Bachelor's Degree in a behavioral health or related human services field and a minimum of one year related field work experience.
Preferred
Bachelor's Degree with three years of experience working with children/families, and/or behavioral health.
Master's Degree with one year experience with children/families and/or behavioral health.
Licensure & Certifications
Valid driver's license.
Computer, copier, facsimile, and shredder equipment use.
Driving record to pass check and ability to drive in day/night and reasonable weather conditions.
