About the job The
Therapeutic Recreation Specialist
under the supervision of the Day Program Manager is responsible for the recreational assessment of participants, development of a plan of care designed to meet the participant’s needs and coordination with other staff, oversight of care planned interventions, management of day center programming and task oversight of activities aides.
Required Qualifications
Degree in Therapeutic Recreation or a related field (equivalent experience may be considered)
At least one year’s experience working with the frail or elderly population
Valid driver’s license, proof of insurance and reliable transportation
Ability to clear Background Check and Drug Screen
Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact
Preferred
Three (3) years’ experience providing therapeutic programming to a frail or elderly population
Essentials
Function as an engaged member of the SCCCO Interdisciplinary Team (IDT), maintaining regular attendance and participating in IDT meetings; communicate participant changes and collaborate on plan of care decisions with the IDT. Attend and participate in Care Team meetings as requested by the Care Manager.
Conduct in‑person initial comprehensive recreational assessments and coordinate with the IDT to develop comprehensive plans of care for each enrolled participant.
Conduct in‑person re‑assessments as needed (semi‑annually, when there is a change of condition/status, or when triggered by the Care Team or IDT).
Manage and develop supportive social, therapeutic, spiritual, cognitive, physical, and recreational programming in the SCC Day Centers that meet participants’ interests, needs, and capabilities. Ensure incorporation of care‑planned interventions into the Day Center programming.
Follow the Therapeutic Recreation philosophy of programming to the highest ability level and assist, through adaptive programming and services, each participant to reach their individual potential.
Provide skilled one‑to‑one therapeutic recreation interventions as indicated by the plan of care.
Advise and support incorporation of therapeutic recreation interventions into home‑care services.
