Physical Therapist Position – Merced, CA We are currently seeking to fill the Physical Therapist Position in Merced and all surrounding areas. This role offers a hybrid model of hourly pay and Paid Per Visit (PPV) with opportunities to accumulate overtime and advance within a merit‑based organization.

Essential Job Functions / Responsibilities
Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
Assess and evaluate therapeutic/rehabilitative/functional status, and participate in the development of the total plan of care.
Direct physical therapy treatment and instruct patients, families, and caregivers in the use and care of therapeutic appliances.
Determine priority needs for physical therapy and report to physicians on patient progress or changes in condition.
Initiate physical therapy programs and instruct family/caregiver members in rehabilitation goals during case conferences.
Train patients in the use of prosthetic devices as needed.
Identify patient and family/caregiver needs for other home health services and refer as necessary.
Prepare and submit clinical progress summaries and clinical discharge plans as directed by organization policy.
Provide in‑service education to nursing personnel and participate in peer consultation.
Supervise physical therapy assistants and home health aide care as per policy and state regulations.
Position Qualifications
Master's or doctoral degree in physical therapy from an accredited program.
Licensed to practice as a physical therapist in California.
One year of appropriate experience as a physical therapist (community/home health experience preferred).
Excellent verbal and written communication and organization skills.
Current CPR certification.
Licensed driver with a valid automobile in good working order and insured per organization requirements.
