Housing Case Manager

The primary responsibility of this role is to deliver comprehensive care and support to enhance the well-being of service recipients. This involves providing therapeutic support and skills training both in person and virtually, fostering communication with clinical professionals and the individual's support system as authorized. Acting as a role model and mentor, the position includes coaching service recipients using approved techniques and strategies and implementing physical intervention when necessary to ensure the safety of individuals. Additionally, meticulous completion of documentation and reporting for grant programs is essential to contribute to the team's operational effectiveness.
Required Education Bachelor's Degree in Health or Human Services.
Acceptable Degrees: Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, and Special Education.
Substitutions OR Bachelor's degree with at least 30 hours of behavioral health coursework.
Required License(s) Valid driver’s license with an acceptable driving record.
Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate.
